It can be continuous or intermittent, deep or burning, and often cannot be explained by a single cause visible on standard examinations. <\/p>\n\n<p>This type of pain can have multiple causes simultaneously and requires a multidisciplinary approach.<\/p>\n\n<h2 class=\"wp-block-heading\"><strong>Possbile Causes<\/strong><\/h2>\n\n<ul class=\"wp-block-list\">\n<li>Post-surgical pain (e.g. after hysterectomy, laparoscopy, prostatectomy, childbirth)<\/li>\n\n\n\n<li>Scar tissue adhesions, fibrosis, or neuropathic pain from local nerve damage<\/li>\n\n\n\n<li>Pelvic floor dysfunction or myofascial pain<\/li>\n\n\n\n<li>Pudendal nerve neuralgia (in some selected cases)<\/li>\n\n\n\n<li>Visceral pain syndrome or chronic nerve irritation<\/li>\n\n\n\n<li>Pre-existing gynecological, urological, or gastrointestinal conditions<\/li>\n<\/ul>\n\n<p>Pelvic pain can have both an inflammatory or mechanical component and a neuropathic component, with alterations in pain perception.<\/p>\n\n<h2 class=\"wp-block-heading\"><strong>How is it Diagnosed?<\/strong><\/h2>\n\n<ul class=\"wp-block-list\">\n<li>Specialist clinical assessment of pain with targeted physical examination<\/li>\n\n\n\n<li>Imaging exams: <strong>ultrasound<\/strong>, <strong>pelvic MRI<\/strong> or <strong>lumbosacral MRI<\/strong> (if indicated)<\/li>\n\n\n\n<li>In some cases: <strong>targeted diagnostic blocks<\/strong> to identify the source of pain (e.g., pudendal nerve, abdominal wall)<\/li>\n\n\n\n<li>Collaboration with gynecologists, urologists, or gastroenterologists if necessary<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\"><strong>How is it Treated?<\/strong><\/h2>\n\n<h3 class=\"wp-block-heading\"><strong>Drug treatment<\/strong><\/h3>\n\n<ul class=\"wp-block-list\">\n<li>Medications for neuropathic pain (e.g., <strong>pregabalin, gabapentin, amitriptyline<\/strong>)<\/li>\n\n\n\n<li>Anti-inflammatories or muscle relaxants<\/li>\n\n\n\n<li>Combined therapies based on the predominant cause<\/li>\n<\/ul>\n\n<h3 class=\"wp-block-heading\"><strong>Physical Therapies and Support<\/strong><\/h3>\n\n<ul class=\"wp-block-list\">\n<li>Pelvic floor physiotherapy (if indicated)<\/li>\n\n\n\n<li>Myofascial therapy and muscle relaxation<\/li>\n\n\n\n<li><a href=\"https:\/\/tidolore.ch\/en\/transcutaneous-electrical-nerve-stimulation\/\" data-type=\"post\" data-id=\"1851\">TENS<\/a> (transcutaneous electrical nerve stimulation) for daily use<\/li>\n<\/ul>\n\n<h3 class=\"wp-block-heading\"><strong>Mini-invasive Interventional Therapies<\/strong><\/h3>\n\n<ul class=\"wp-block-list\">\n<li><strong>Targeted ultrasound-guided injections<\/strong> (e.g., abdominal wall blocks: <strong>TAP block<\/strong>) <\/li>\n\n\n\n<li><strong>Selective pelvic nerve blocks<\/strong> (ilioinguinal, genitofemoral, pudendal)<\/li>\n\n\n\n<li><strong>Pulsed radiofrequency<\/strong> of the involved nerves, in resistant cases<\/li>\n\n\n\n<li><strong>Injections on painful scars<\/strong> (post-cesarean, laparotomy, episiotomy)<\/li>\n\n\n\n<li><strong><a href=\"https:\/\/tidolore.ch\/en\/spinal-cord-neurostimulation\/\">Spinal cord neuromodulation<\/a><\/strong><\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\"><strong>Prognosis<\/strong><\/h2>\n\n<p>Chronic pelvic pain is a complex condition, but <strong>manageable with a personalized approach<\/strong>.
