{"id":3419,"date":"2025-07-14T13:37:09","date_gmt":"2025-07-14T13:37:09","guid":{"rendered":"https:\/\/tidolore.ch\/staging\/complex-regional-pain-syndrome-crps-algodystrophy\/"},"modified":"2026-03-17T17:49:36","modified_gmt":"2026-03-17T17:49:36","slug":"complex-regional-pain-syndrome-crps-algodystrophy","status":"publish","type":"post","link":"https:\/\/tidolore.ch\/staging\/en\/complex-regional-pain-syndrome-crps-algodystrophy\/","title":{"rendered":"Complex Regional Pain Syndrome (CRPS, Algodystrophy)"},"content":{"rendered":"\n<p>The <strong>Complex Regional Pain Syndrome (CRPS)<\/strong> is a rare disease of the nervous system that causes <strong>intense and prolonged pain<\/strong> in a part of the body (usually <strong>a hand, an arm, a foot, or a leg<\/strong>) after an injury, surgery, or even just after minor trauma (such as a sprain or fracture).<\/p>\n\n<p>In <strong>CRPS Type I<\/strong>, there is no visible direct nerve injury. Despite this, an abnormal reaction of the nervous system and local inflammation develops. <\/p>\n\n<h2 class=\"wp-block-heading\">Why Does it Develop?<\/h2>\n\n<p>It is not yet completely clear <strong>why some people develop CRPS<\/strong>. Research suggests that several factors are involved: <\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Alteration of the autonomic nervous system<\/strong> (which controls sweating, temperature, heart rate, etc.)<\/li>\n\n\n\n<li><strong>Abnormal local inflammation<\/strong> that persists over time<\/li>\n\n\n\n<li><strong>Hyperactivation of sensory nerves<\/strong>, which become overly sensitive<\/li>\n\n\n\n<li><strong>Modifications of the central nervous system<\/strong> (the brain misinterprets pain signals)<\/li>\n<\/ul>\n\n<p>In other words: <strong>it is as if the body does not &#8220;turn off&#8221; the pain alarm<\/strong>, even though the initial injury has healed.<\/p>\n\n<h2 class=\"wp-block-heading\">What are the symptoms?<\/h2>\n\n<p>CRPS usually manifests <strong>within 4-6 weeks of trauma<\/strong> or surgery. Symptoms vary over time and from person to person: <\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Very intense pain<\/strong>, burning or stabbing, disproportionate to the initial cause<\/li>\n\n\n\n<li><strong>Red, cold, or hot skin<\/strong>, sometimes with abnormal sweating<\/li>\n\n\n\n<li><strong>Persistent swelling of the limb<\/strong><\/li>\n\n\n\n<li><strong>Weakness and difficulty moving the affected part<\/strong><\/li>\n\n\n\n<li><strong>Hyperesthesia<\/strong>: even a light touch can cause pain<\/li>\n\n\n\n<li><strong>Changes in nails and hair<\/strong> (slow growth, brittle nails)<\/li>\n\n\n\n<li>In advanced stages: <strong>joint stiffness, loss of muscle mass<\/strong>, contractures<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">How is the diagnosis made?<\/h2>\n\n<p>The diagnosis is based on the <strong>Budapest Criteria<\/strong>, a set of clinical signs and symptoms. There are no specific blood tests. Sometimes supportive examinations are used to exclude other pathologies:  <\/p>\n\n<ul class=\"wp-block-list\">\n<li><strong>Magnetic resonance imaging<\/strong> or <strong>bone scintigraphy<\/strong><\/li>\n\n\n\n<li><strong>Neurological evaluation<\/strong><\/li>\n<\/ul>\n\n<p>It is important to receive an <strong>early diagnosis<\/strong>, as treatment is more successful in the initial stages.<\/p>\n\n<h2 class=\"wp-block-heading\">How is it Treated?<\/h2>\n\n<h3 class=\"wp-block-heading\">Multimodal Treatment: the most Effective Method<\/h3>\n\n<p>CRPS <strong>has no definitive cure<\/strong>, but <strong>can improve or stabilize<\/strong> with targeted, early, and multidisciplinary treatment.<\/p>\n\n<ol class=\"wp-block-list\">\n<li><strong>Medications<\/strong>:\n<ul class=\"wp-block-list\">\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Anti-inflammatory drugs (in the early stages)<\/span><\/li>\n\n\n\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Antidepressants and anticonvulsants for neuropathic pain (e.g., amitriptyline, gabapentin, pregabalin) <\/span><\/li>\n\n\n\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Corticosteroids in acute phases<\/span><\/li>\n\n\n\n<li>50% topical DMSO (in selected cases)<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Physical therapy<\/strong>:\n<ul class=\"wp-block-list\">\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Gentle and progressive mobilization of the limb<\/span><\/li>\n\n\n\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Desensitization (to reduce hypersensitivity)<\/span><\/li>\n\n\n\n<li>Occupational therapy to promote daily autonomy<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Psychological support<\/strong>:\n<ul class=\"wp-block-list\">\n<li>To help cope with chronic pain, stress, and fear of movement<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Interventional therapies (in resistant cases)<\/strong>:\n<ul class=\"wp-block-list\">\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">Sympathetic blocks (injections near autonomic nervous system nerves)<\/span><\/li>\n\n\n\n<li><span style=\"color: initial; font-family: -apple-system, BlinkMacSystemFont, \" segoe=\"\" ui=\"\" roboto=\"\" neue=\"\" arial=\"\" sans=\"\" sans-serif=\"\" color=\"\" emoji=\"\" symbol=\"\">In some selected and resistant cases, low-dose intravenous ketamine infusions may be considered, under specialist monitoring<\/span><\/li>\n\n\n\n<li><a href=\"https:\/\/tidolore.ch\/staging\/en\/spinal-cord-neurostimulation\/\">Neuromodulation<\/a> (spinal stimulation in selected cases)<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n<h2 class=\"wp-block-heading\">The Importance of Collaboration<\/h2>\n\n<p>Treatment success depends on <strong>teamwork<\/strong> between:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Pain medicine physician<\/li>\n\n\n\n<li>Physical therapist<\/li>\n\n\n\n<li>Clinical psychologist<\/li>\n\n\n\n<li>Possible orthopedic or neurological specialists<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">What is the Prognosis?<\/h2>\n\n<p>CRPS can be a complex condition, but many patients show improvement over time, especially if the diagnosis is early and treatment is personalized.
